SNDA: $200 Early Bird (Until March 9), $230 (Until May 31st), $260 (After May 31st & Onsite)

 

USNDA: $175

 

CANCELLATION: A full refund will be granted only if you cancel by June 15th. A 50 percent refund will be granted if you cancel by June 30th. After June 30th, no refunds will be honored. All requests must be sent in writing via email to the SNDA Treasurer Sara Mahmood.





CONVENTION REGISTRATION INCLUDES admission to all of the following organization events:

^ Admission to all SNDA and NDA professional developmental sessions
^ Access to all SNDA and NDA technical and vendor exhibits
^ One (1) Joint Conference T-shirt/Gift
^ Admission to all SNDA Night on the Town Events
^ Two (2) Plated Breakfasts and One (1) lunch
^ Delegate participation in all voting sessions
^ Scientific Poster and Scrapbook competition
^ One (1) ticket to SNDA’s Gala. Inclusive of "Rites of Passage", "Presidential Awards", Plated Dinner
^ Admission to all SNDA Night on Town events

(1) A separate registration form must be submitted for each individual.

(2) Name badges must be worn for all SNDA and NDA activities. No one will be allowed to attend any SNDA or NDA function without a badge.

(3) Registration will not be considered complete unless all information is properly entered and payment complete. SNDA or USNDA membership must be current or paid with this form.

Annual Membership Dues: $45.00  before November 15, 2012 at 11 11:59pm.  $55.00 afterwards

(4) The appropriate registration fee should be postmarked by June 30, 2014.
